Professional Volunteer Registration
All health professional volunteers providing direct patient care
MUST provide their own liability coverage.
This form is for Medical/Dental Professionals only:
- Dentists: General and Specialist Dentists Needed (Endodontics, Oral Surgery, Pediatric Dentistry, Prosthodontics, Periodontists)
- Registered Dental Hygienists: Needed for assistance in Hygiene, Patient Education and Radiology areas
- Dental Assistants
- Pharmacists
- Dental Lab Technicians
- X-Ray Technicians
- Medical Professionals: Physicians, Nurse Practitioners, Physicians' Assistants, Registered Nurses, EMTs
The CCRI dental clinic has fully functioning operatories. Disposable dental supplies, dental materials, and some handpieces and
instruments will be provided. To make your experience more comfortable and enjoyable, please feel free bring your own LATEX FEE
gloves, and STERILE instruments, labeled with your name. Handpiece tubing compatibility is as follows:
- Dental Assisting Lab (Restorative Area): 6 hole power optic (bulb in handpiece) and 5 hole fiber optic (bulb in handpiece)
- Dental Hygiene Clinic (Hygiene, Oral Surgery, Endodontic Areas): 4 hole
